We have learned what the word documentation means, however, if someone mentions this word to you and does not show you how to do it properly, it is just another word to you. Let us correct that right now, ready.
- Change your thinking right now writing down fact needs to become a habit. Everyday facts that most people would not even think twice about, automatically we all think that we will remember. If you get nothing else out of this documentation review, retain this statement write it down.
Never rely on memory yours or anyone else’s memory is not reliable.
Write down things like dates, time of day, persons involved, synopsis of conversations taking place. Remember the 4W’s, who what, where, and why. How, a description of the incident, names of witnesses.

Scenario; One morning you took Sarah to the local fair in the Huntington Mall parking lot. Your husband had left for work early for an important meeting for clients flying in today, he left around 7:15you wanted to get an early start. Since the health problem developed about seven months ago Sarah tires easily.
Arriving at the fair, looking around for a safe ride she saw the merry go round, excited and jumping up and down, she asked if she could go on the merry go round, you decided to go on with her for safety reasons. Waiting in line seemed to take forever, eventually you gave you tickets to the attendant, wow it was Jimmy Bullock and two other of your local volunteer firemen were all helping out at this fund raising event. Sarah ran ahead while you were saying goodbye to Jimmy, you hurried to catch up with her.
While stepping up on the platform, Sarah slipped on the side of the platform and bumped her knee. Sam Preston who was heading for the controls observed her fall. He brought his first aid kit and asked Sarah to sit on the side of the merry go round while he looked at her knee. Sure enough, there was a little skinned area on her knee, you could tell he had experience as he has four children; he gently cleaned it off and placed a cool Nemo bandage over the injured area. All was well, and Sarah climbed up onto one of the horses, with the assistance of Dennis Smith the other fireman.
Before we realized it two hours had passed, and we were ready to head for home.
